Title: [abstract] HIV-RELATED FATIGUE AND HYPERBARIC OXYGEN THERAPY.

Abstract: Hyperbaric Oxygen Therapy (HBO) was evaluated to determine its effectiveness in relieving HIV-related fatigue and its effect on immune function. Sixteen pts. with confirmed HIV, CD4 counts of 500 or less, ages 18-65 yrs. with complaint of fatigue and no active opportunistic infections were treated with 100% oxygen at 2 ATA for 90 minutes three times per week for 6 months or longer. Five pts. were enrolled in a control arm and treated initially with surface air (blinded) for two weeks to rule out a placebo effect. These pts. were then treated with 100% oxygen for the remainder of the study. Laboratory evaluations included CD4, CD8, Hgb/Hct, and Tumor Necrosis Factor (TNF) bi-weekly. Clinical evaluations, weight, and subjective fatigue levels were evaluated weekly. Results indicated relief of fatigue within two weeks for all pts., stable or weight gain, stable or increase in CD4 and Hgb/Hct levels, and inhibition of TNF. (TNF is a cytokine which has been linked to fatigue, weight loss, fever, and disease progression in HIV/AIDS. All pts. had significant levels of TNF, except for three for whom baselines were unavailable, at baseline which became undetectable after HBO therapy). Further, 14/16 pts. had no clinical progression of disease which would be expected in this group of which 80%, have 40 CD4 cells or less. HBO may be effective in relieving fatigue and improving quality of life of pts. with HIV/AIDS by inhibiting production of TNF. In the medical management of HIV/AIDS, HBO may reduce hospitalizations and associated costs. Further research is warranted in view of these encouraging results.
