Abstract: BACKGROUND: it is still an unresolved orthpedical problem that of FHN treatment. The proposed solutions (osteothomy, core decompression, bone grafting, electrical stimulation) are not fully satisfying, while total joint arthroplasty is often not advisable because of patients' young age. METHODS: 35 patients were included in our study (27 males and 8 females) whose average age was 46.9 (21-78). Diagnoses were' made both clinically, that is deambulation with crutches, spontaneous pain, hip mobility reduction, and by imaging: 14 patients underwent MNR, 20 Tc 9m scintigraphy, 1 patient underwent CT scanning. All the tests were repeated after one year of hyperbaric treatment. The patients included in our study belonged to class II (N=27) and III (N=8) of Ficat's classification. The average number of hyperbaric treatments per year was 107 (2.5 ATA 60' FiO2 =100, air compressed chamber), divided as follows: 30 for the first cycle and 20 for the following cycles. RESULTS: 31 patients over 35 recovered (88.5%). Of those 27 (77.1%) showed both a clinical and imaging recovery, 4 (11.4%) showed clinical recovery and no imaging modification. 4 patients showed no clinical recovery nor imaging modification. The 27 recovered patients belong to Ficat's class II, while the remaining 8 belong to class III. Follow-up > 36 months favourable for 14 patients, 26-36 months favourable for 12 patients, < 24 months favourable for 5 patients. CONCLUSIONS: considering the results in literature we think that HBO is the most effective therapeutic approach to FHN when precociously applied and when consisting of at least 90 treatments in one year, especially for Ficat's class II.
