Title: [abstract] MANAGEMENT AND TREATMENT OF HYPERBARIC OXYGEN INDUCED SEIZURES

Abstract: BACKGROUND: Central nervous system (CNS) oxygen toxicity with subsequent seizure activity during hyperbaric oxygen therapy (HBOT) is a rare yet known complication. All patients who undergo hyperbaric treatment are routinely informed of this risk as a part of their treatment informed consent process. Although this is a known and self-limiting event, the treatment standards of HBOT induced seizures vary across hyperbaric treatment centers. There is currently no national consensus on the work-up and treatment of these seizures. MATERIALS AND METHODS: A total of 82 hyperbaric centers approved by the Accreditation Council of the Undersea & Hyperbaric Medical Society were surveyed inquiring about their current practices when confronted with and treating hyperbaric induced seizures. The HBOT seizure survey addresses individual centers' approach to the seizure patient. Centers were asked to answer twelve questions regarding their current practice related to HBOT induced seizures. RESULTS: Sixty-three percent (63%) of the hyperbaric centers responded to the survey. Of these centers, 94% have a seizure protocol and 61% of patients who had a seizure continued with HBOT. Of those who had a seizure, 7% were started on anti-seizure medications and dilantin was the drug of choice. Of those who had a seizure, 27% had neurologic consultations. Neurologic recommendations included: initiation of anti seizure medications 0.9%, continuation of treatment without intervention 1.8%, recommendation to hold treatments 3.7%, and request for neuroimaging 6.4% of the time. CONCLUSIONS: Most patients continue with HBOT in the presence of CNS oxygen induced seizures. Although neurologic consultation was requested one quarter of the time, minimal intervention was done and pharmacologic therapy was rarely initiated. Although there is no consensus, a similar approach appears to be used by most approved hyperbaric centers. A consensus panel on this topic for creation of a position statement may be helpful in guiding future care of these patients.
