Abstract: BACKGROUND: SCUBA divers are not only subject to decompression stresses but to juxta-diving physical stresses. At times it may be difficult to differentiate the cause of symptoms from these two sources after SCUBA diving. Although suggestions how to differentiate physical injury from decompression sickness (DCS) are mentioned in the diving literature, we were unable to locate a specific case report describing the concomitant occurrence of musculoskeletal decompression sickness (pain only bends) and a musculoskeletal (MS) injury. METHODS: We present a case report of a SCUBA diver who presents with pain only DCS and concomitant ligamentous sprain symptoms of his knee after two omitted decompression divers and a post-dive horse back ride. The knee symptom occurred while horse back riding 3 hours after the SCUBA dives were completed. Two distant pain symptom locations and pain characteristics were noted. Only the ligamentous injury was tender to palpation. first hyperbaric oxygen (HBO) recompression treatment, the knee ligament sprain injury symptoms, although not as rapid to respond, appeared to be markedly benefited by the HBO therapy. CONCLUSIONS: It is important for the hyperbaric medicine physician to differentiate between MS injury and DCS symptoms since the diving environment can cause either or both to occur. In such situations a trial of recompression with HBO should always be one. From the medical assessment and responses to HBO, appropriate diagnoses and logical management can be determined. We provide an algorithm to show our approach for the diver who presents with possible concomitant pain only DCS and a MS injury in the same joint.
