Title: [abstract] HYPERBARIC OXYGEN THERAPY IN THE TREATMENT OF RADIATION INDUCED CNS NECROSIS.

Abstract: Radiation induced CNS necrosis has been reported in patients receiving "boost" irradiation (brachytherapy or stereotactic radiosurgery) for primary and recurrent treatment of both CNS malignancies and benign condition. However, the treatment of this radiation induced CNS necrosis has been limited to the use of steroids and neurosurgical intervention. We report on three patients who received hyperbaric oxygen therapy for CNS necrosis and detail their results. Between October of 1991 and November of 1993, 3 patients with biopsy proven (n=2) and/or with neuroimaging changes suggestive of radiation necrosis (n = 1) received HBOT following no response to steroid therapy. Two patients had received iodine-125 implantation for boost therapy and a single patient received stereotactic radiosurgery treatment for an arteriovenous malformation. Time to development of completion of boost therapy ranged from 4 to 12 months following an initial trial steroid therapy with doses up to 16 mg per day without evidence of resolution on MRI scans; patient were referred for hyperbaric oxygen therapy. Patients received between 25 and 30 treatments at 2-2.4 atmospheres for 90 minutes. In followup, which ranges between 3 and 27 months, each patient has had documented resolution of neuroimaging changes noted on MRI and have had significant improvement of Karnofsky performance status. Although more followup is required, this early experience has demonstrated significant benefit with improved patient Karnofsky performance status and warrants further collaborative efforts to determine if hyperbaric oxygen should be included in therapy for radiation induced CNS necrosis.
