Abstract: No knowledge is possessed of the structural basis of the blood-brain barrier fragility during acute decompression sickness (DCS). The permeability of microvasculature in the cerebral cortex, neostriatum and spinal cord to intravenously injected horse-radish peroxidase (HRP) has been investigated in the rat after experimental compression to 5,1 bar air for 90 min, and subsequent decompression to the ambient pressure in 1 min. In another series of experiments, for comparison 1 ml 2,0 mol/l urea was infused into the right common carotid artery of rats under Nembutal anaesthesia during 1 min. After compression-decompression exposure for 15 min focal leaky areas were found under the light microscope in all the regions examined. The leakage was most prominent in the gray matter of the spinal cord, and the cerebral cortex immediately beneath the pia. Adjacent to the leaky areas pericytes, and especially in the cerebral cortex, some neurons accumulated the trace. Under the electron microscope, HRP accumulated in the basement membrane of the leaky microvessels in both decompressed and urea-treated rats. The number of HRP-containing granules in the endothelium was increased as compared with the controls. In conclusion, acute DCS induces focal leakage of exogenous HRP to the brain and spinal cord, closely resembling leakage induced byhyperosmolar infusion.
