Abstract: BACKGROUND: In the last four years, we have treated 22 patients referred to our units for adjunctive hyperbaric treatment for management of Crohn's disease fistula. Twenty patients (91percent) had multiple perineal fistulae (one female patient had one between vagina and rectum, and a young male had three in the abdominal wall). The duration of the fistulae prior to HBO treatment was from 6 months to 3 years (mean 32 months), and the fistulae were refractory to conservative and surgical treatments. HBO was applied during the first five days QID at 2.7 atm abs for 2 hours, and then every 48 hours, until clinical healing was accomplished. RESULTS: This type of patient has a tendency to have a poor adherence to treatment and 45percent (10 patients) of our population interrupted treatment before completion of healing, after showing varying degrees of improvement. Two patients are still being treated. Of those who completed the treatment - 10 patients - they underwent complete healing after a mean of 29 treatments (range 14 to 48). There were no deaths in our patients. CONCLUSIONS: 1) HBO could be an effective adjunctive treatment for refractory Crohn's disease fistulae due to its antimicrobial and healing effects on hypoxic chronic problem wounds. 2) Patients have to be persuaded to complete the HBO protocol to accomplish healing. 3) This type of patient is very resistant to adhere to any treatment modality due to the poor results obtained with conventional conservatory and surgical treatments.
