Abstract: BACKGROUND: Diabetic foot ulcers are associated with significant healthcare costs as well as significant considerable mortality and morbidity. In our demographic we encounter many patients with limb and life threatening diabetic foot ulcers. Our study encompassed the use of ionic silver in conjunction with a course of hyperbaric oxygen in the diabetic foot classified as Wagner 3 or greater. MATERIALS AND METHODS: Patients were selected that had a diabetic foot ulcer Wagner 3 or greater with active infection. Patients were seen daily five times per week for hyperbaric oxygen treatments and after their daily treatment was completed their wounds were dressed with ionic silver powder. Three patients were selected for this paper. RESULTS: Amputation was averted in all three patients. All three went on to either heal completely or were ready for autologous skin grafting. Wound bed preparation was successful; there was full granulation over bone noted.
