Abstract: BACKGROUND: FLA is a frequent and disabling condition with no effective treatment, clinically associated with disturbances of gaie, incontinence, rigidity and cognitive impairment (Neurology, 1994; 44:1246-52). Data presented is part of a prospective, patient-blinded, controlled study of HBO in vascular FLA. METHODS: Patients received ten daily sessions of HBO at 2.5 ATA for 45 minutes, or air at 1.1 ATA. Before and after therapy, to assess cognitive deficits, we performed CAST (J Neurol Sci 1997; 150 (Supp:294) (observer indedependent) and FMMS (J Psychiatric Res 1975; 12: 189-198). RESULTS: There were 25 patients in the HBO group and 11 patients in the control group. The groups were similar clinically and with respect to baseline FMMS and CAST scores. A significant clinical improvement lasting up to 6 months occured in the HBO group but not in the control group. In CAST, the HBO group decreased delays from 18.3 + 21.6 to 10.1 + 17.3 (p less than 0.003) and omissions from 8.67 + 10.9 to 4.39 + 7.14 (p less than 0.003). Retest delays decreased from 18.1 + 18.5 to 11.2 + 16.4 (p less than 0.058) and omissions decreased from 13.2 + 12.3 to 5.6 + 7.5 (p less than 0.0004) [mean + SD] (Wilcoxon). FMMS scores increased from 24.8 + 2.7 to 26.8 + 2.39 (p less than 0.001) [mean + SD] (Student t-test). No significant improvement was observed in the control group. CONCLUSIONS: HBO reduces cognitive deficit in vascular FLA, indicating the existence of a previously unknown functional reserve in this patient population.
