dc.description.abstract Introduction/Background: The treatment of cancer patients with a history of radiotherapy to the mandible with adjunctive HBO2T to prevent ORN of the mandible has been performed for over 20 years. Some radiation oncologists and oral surgeons have argued that HBO2T is no longer necessary due to the evolution of more efficient radiation and surgical techniques. We compared two surveys of those in both radiation oncology and hyperbaric medicine regarding the use of HBO2T for ORN prevention. Materials and methods: Separate web-based anonymous surveys (email invitation) were taken of ASTRO(American Society for Therapeutic Radiology and Oncology) members and UHMS (Undersea and Hyperbaric Medicine Society) members. Results: A total of 293 surveys were completed by physicians (118 UHMS and 175 ASTRO). 64 percent of ASTRO members vs. 82 percent of UHMS members recommend HBO2T for the prevention of ORN. Among those not recommending HBO2T, 52 percent vs. 38 percent respectively cite a lack of evidence to support its use, while 25 percent vs. 71 percent respectively cite a lack of support from colleagues. 79 percent of ASTRO members and 85 percent of UHMS members believe it is important that a new effectiveness RCT is undertaken to “measure treatment efficacy” (96 percent vs. 90 percent) and “gain insurance approval” (89 percent vs. 92 percent). Summary/Conclusion: Current evidence in support of HBO2T for the prevention of ORN is taken from a single RCT published by Marx et al. in 1985 and several case series. The overall incidence of ORN has declined since the introduction of modern radiation therapy techniques, while there has also been a relative decrease in the incidence of ORN in modern-era (IMRT patients) treated with HBO2 (i.e., less than 2 percent vs. Marx study of 5.4 percent ). A new multicenter RCT on the effectiveness of HBO2T for the prevention of ORN is indicated and would have overwhelming support from both radiation oncology and hyperbaric medicine. en_US
